I'm 5'4" 110 lbs. size 4. Do I need the small or universal size?

The small measures: 12.75x19x12.25
the Universal measures: 14x23x12.5

When this pad is attached to the Iceman machine, will it provide compression as well as ice?

No this will only provide contiuous cold therapy. Our Aircast Cryocuff IC is the only unit that provides cold therapy (not continuous) and compression https://www.donjoystore.com/aircast-cryo-cuff-ic-motorized-and-cuffs

You could but it will not wrap around very well. I would suggest the Universal Pad
