Most people don't abandon a knee brace because it stopped supporting the knee. They abandon it because it got hot, itched, slid down twice an hour, or showed through their work pants. The support was fine. The material was wrong.

Neoprene and knit are the two most common materials in knee bracing, and they behave very differently on the leg.

First, a warning about shopping by material

Material is a comfort decision, not a support decision.

If you have an ACL tear, a recent surgery, or instability that makes the knee give out, the support comes from the frame, the hinges, and the strapping system. A comfortable sleeve will not do that job no matter what it's made of. Pick your support level first, based on the injury and what your surgeon or physical therapist told you. Then use material to decide between the options at that level.

For a lot of shoppers, that narrows things quickly. Rigid functional braces and unloader braces usually come with a padded liner and limited material choice. The neoprene vs. knit question matters most in sleeves, wraparound supports, and hinged soft braces, where the fabric is the brace.

What neoprene actually is

Neoprene is a synthetic rubber. In bracing it's used as a closed-cell foam, usually laminated with a thin nylon or polyester jersey on one side or both. That construction is why neoprene braces feel substantial in your hand and why they hold their shape when you take them off.

Closed-cell means the foam does not let air or water pass through it, which drives most of how the material behaves.

It traps body heat against the knee, raising the temperature of the joint and the surrounding soft tissue. Many people with arthritis or chronic stiffness find that warmth helpful, especially in cold weather or first thing in the morning.

It compresses well and stays compressed. The foam pushes back against the leg all day instead of relaxing out after a few hours.

It grips. Neoprene has natural friction against skin, so it resists sliding down the leg better than most smooth fabrics.

It's also thick, typically 3mm to 6mm, so it adds noticeable bulk under clothing.

What knit actually is

Knit braces are made from engineered fabric, usually nylon or polyester blended with elastane or spandex. The knit is often zoned, meaning the manufacturer varies the tightness of the weave across the brace so it compresses more over the joint and less behind the knee.

Air passes through it. Sweat moves through it and evaporates off the outside surface.

Knit is thin and low profile. It disappears under jeans and dress pants in a way neoprene does not.

The compression is real but it behaves differently. Knit gives you elastic compression that moves with the leg rather than the firm, static squeeze of foam. Some people describe it as lighter. Others say it feels like it's doing less, even when the measured compression is similar.

Knit fabric also relaxes over the course of a day and over the life of the brace. A knit sleeve you've worn daily for months will not compress the way it did new.

Heat and sweat

Neoprene holds in heat and holds in moisture. If you're on your feet for an eight hour shift, working outside in summer, or exercising hard, sweat has nowhere to go. It pools between the brace and the skin. The brace gets heavy, the fit changes as it gets saturated, and it starts to smell unless you wash it often.

Knit lets that moisture move. It won't keep you dry the way a technical running shirt does, but you'll notice the difference by hour six.

The reverse case matters too. If your knee aches in the cold, if you have osteoarthritis that flares in winter, or if you use your brace mainly for morning stiffness, neoprene's heat retention is the point. Knit will feel thin and unsupportive by comparison.

Perforated neoprene is a middle option. The manufacturer punches small holes through the foam to let air move. It breathes better than solid neoprene and worse than knit, and it keeps most of neoprene's grip and firmness.

Skin sensitivity

Two different problems get lumped together here, and they have different solutions.

The first is irritation from trapped sweat and friction. Skin stays damp under an occlusive material for hours, softens, and then rubs. You get redness, small bumps, itching, sometimes a rash in the crease behind the knee. This is the more common complaint, and it's mostly a heat and moisture problem. Switching to knit, or to a perforated or moisture-wicking material, usually resolves it. So does taking the brace off periodically through the day and letting the skin dry out.

The second is allergic contact dermatitis. This is a true immune reaction, and with neoprene the trigger is usually not the neoprene itself but the chemical accelerators used to manufacture it, most often thiurams and carbamates. The reaction shows up as a well-defined rash matching the exact outline of the brace, often a day or two after wearing it, and it does not improve just because you dried the skin. If that's what's happening, no amount of ventilation fixes it. You need a neoprene-free brace.

Worth knowing: neoprene is not natural rubber latex, so a latex allergy does not automatically rule out a neoprene brace. Other components might contain latex, though, so check the product specifications if you have a diagnosed latex allergy.

If a rash persists after you switch materials, or if the skin breaks open, see a clinician rather than working through the catalog.

Wearing it all day

What does an average day with the brace look like? That usually settles it.

For desk work and commuting, nine hours under work clothes, go knit. It's cooler, it's invisible under trousers, and it doesn't fatigue you the way a thick foam brace does over a long sitting day. Watch for migration, though. Knit sleeves slide down the leg more than neoprene unless they have a silicone grip band at the top.

For standing, walking, lifting, and outdoor work, temperature matters more than the activity. Knit in heat, neoprene in cold.

For training sessions of an hour or two, neoprene handles the load well and the heat matters less over a short session. Its grip also holds up better against repeated flexion. Same answer for nighttime and early morning stiffness, where you want the warmth.

Two situations point to a wraparound with adjustable straps regardless of material. One is swelling that changes through the day, since you can loosen the straps as the leg changes. The other is arthritis in the hands or trouble bending down, because pull-on sleeves are the hardest style to get on.

Care and how long they last

Neoprene needs cold water, mild soap, and air drying. Heat is what kills it. A hot dryer or a sunny windowsill breaks down the foam, and once it stiffens or starts separating from the jersey lining, it's finished. Fabric softener also degrades it.

Knit is easier. Most knit braces can be machine washed on a gentle cycle in a mesh bag and hung to dry. They tolerate frequent washing better, which matters if you're wearing one every day. They wear out by losing elasticity rather than by falling apart, so the sign is that the sleeve stops staying put.

On replacement timing, DonJoy®'s own guidance is to swap a knee brace at least once a year if you wear it every day, and up to every two years if you only pull it out for occasional sport. Watch for wear rather than the calendar. The brace is done when it no longer compresses or stays where you put it.

Owning two braces and alternating them stretches the life of both, since each one gets a full day to dry out.

The short version

Choose neoprene if you want warmth, you want firm compression that doesn't fade through the day, you need adjustable wraparound straps, or your use is short sessions rather than all-day wear.

Choose knit if you run hot, you're wearing the brace all day under clothes, sweat has caused you skin problems before, or you want something you'll actually remember to put on.

Choose a neoprene-free material like Drytex if you've had a reaction that matched the outline of a previous brace.

And if you're stuck between two braces at different support levels, the support level wins. A brace you find slightly too warm still protects the knee. One that's too light doesn't.
