How to Treat Patellar Tendonitis: Bracing, Rest & Recovery

That sharp, nagging ache just below your kneecap, the one that flares up when you squat, take the stairs, or push off the ground, is a signature sign of patellar tendonitis. It is one of the most common overuse injuries in the active population, and it has a habit of getting worse the longer you ignore it.

Patellar tendonitis is especially common in athletes who rely on high impact activities or do lots of, repetitive jumping. At any given time, up to 45% of elite jumping athletes may be experiencing symptoms.1

The prevalence is particularly high in sports like basketball and volleyball, where studies estimate rates of 45% and 32%, respectively.1

The good news: patellar tendonitis responds well to the right combination of rest, targeted support, and a commitment to addressing what caused the patellar tendonitis in the first place. This guide, written by a certified athletic trainer, walks you through exactly what that looks like.

What Is Patellar Tendonitis?

The patellar tendon runs between your kneecap (patella) and the top of your shin bone (tibia). Its job is to transfer force from your quadriceps muscles down through your knee.Every time you jump, run, or flex the quadriceps muscles, that tendon is working hard. Patellar tendonitis, sometimes called jumper’s knee, happens when repetitive stress creates small tears in that tendon, leading to inflammation, pain, and tenderness right at the base of the kneecap.

This is not the same as a ligament sprain or an acute knee injury. It is a use-related condition, which means it builds gradually and, crucially, can get significantly worse if you train through it without making changes.

Symptoms and What Makes Patellar Tendonitis Worse

The pain from patellar tendonitis is localized and usually easy to identify. You will feel tenderness directly on the tendon between the kneecap and the shin. Not diffuse joint pain, but a specific sore spot you can almost point to with one finger.

Activities that commonly aggravate symptoms include:

  • Jumping and landing
  • Running, especially downhill or on hard surfaces
  • Squatting and lunging
  • Climbing or descending stairs
  • Extended periods on your feet

Left untreated, patellar tendonitis can progress from activity-related discomfort to persistent pain during everyday movement. In more serious cases, repeated stress can lead to a partial or full tendon rupture. In younger athletes whose bones are still developing, patellar tendonitis left untreated, can lead to a condition called Osgood-Schlatter disease, where the tendon pulls on the growth plate of the shin.

Most cases of patellar tendonitis, especially when caught early, can be managed conservatively. The key is to treat the root cause, not just the symptoms2.

  • Rest and Activity Modification

    The moment you notice patellar tendon pain, your first priority is reducing the load on the tendon. That means dialing back or temporarily stopping the activities driving the irritation.Continuing to push through patellar tendonitis adds more micro-tears to a tendon that’s already struggling to keep up. Your physician may recommend a structured rest period before you return to activity.

  • Address Muscle Imbalances and Flexibility

    Patellar tendonitis rarely appears in isolation. Weak quadriceps, tight hamstrings, or an imbalance between the two are common contributors since they shift excess load onto the tendon during movement. A targeted stretching and strengthening routine that addresses these muscle groups is one of the most important parts of recovery.

  • Evaluate Your Movement Patterns

    How you run, jump, or land also matters. Poor mechanics during high-impact activities can place repeated stress on the patellar tendon even at moderate training volumes. If form is contributing to your injury, no amount of rest will fully solve the problem until you address it.

Do Knee Braces Help with Patellar Tendonitis?

Yes, with an important caveat. Bracing can be a genuinely useful part of managing patellar tendonitis3, but it works best as a support tool alongside the real treatment, not as a replacement for it.

Because there are small tears in the tendon, using a brace to mask pain and continue at full intensity can lead to more damage. A brace is used to offer additional support during your recovery period, not a tool to help ignore the injury.

When used correctly, bracing may provide several benefits:

  • During the rest period, targeted compression can help reduce swelling and soreness during daily activities.
  • When returning to activity, a brace applies direct pressure to the patellar tendon, redistributing load and providing stability as you rebuild strength.
  • Bracing can offer some confidence when returning to activities, especially during the early weeks where caution is needed.

Choosing the Best Knee Brace for Patellar Tendonitis

Not all knee supports work the same way, and the right choice depends on where you are in recovery and what your symptoms involve.

Patellar Tendon Straps

Many athletes choose a patellar tendon strap during early return-to-sport. A patellar tendon strap (sometimes called an infrapatellar strap) applies targeted pressure to the tendon, which can alter the angle of force transmission and reduce strain at the insertion point on the tibia. A patellar tendon strap sits directly over the tendon between the kneecap and the shin. It applies focused compression to the injured site, which helps redistribute the load on the tendon during movement.3

This is the go-to option when:

  • Pain is localized directly on the patellar tendon
  • There’s no significant swelling of the surrounding joint
  • You need a low-profile option that fits under athletic gear

The strap should feel snug enough to apply noticeable pressure on the tendon but not so tight that it affects circulation or creates pressure points behind the knee. You will find your preferred tension within the first few days of use.

Supportive Knee Braces

A full supportive knee brace covers more of the joint and is a better fit when patellar tendonitis is accompanied by other issues like joint swelling, a concurrent ligament sprain, or significant quadriceps strain. It is bulkier than a tendon strap, which may be a factor in brace selection depending on your sport or activity, but it offers broader benefits: ligament support, compression across the whole joint, and more structural confidence during higher-impact movement.

This is the preferred option for returning to high-impact activities when you need more than just tendon-specific support.

Compression Sleeves

Compression sleeves can help reduce joint swelling associated with patellar tendonitis, and they provide a general sense of support during activity. However, they do not apply targeted pressure to the patellar tendon itself, so if tendon-specific support is what you need, a strap will serve you better.

Sleeves are a reasonable option for mild swelling management during rest and recovery, or as a light support layer when symptoms are minimal.

How to Wear a Brace During Recovery

Fit matters as much as the type of brace you choose. Here are a few practical guidelines:

  • Position a patellar strap directly over the tendon, between the kneecap and the shin, not above or below it.

  • Muscles swell during activity, so leave a little room if you’re bracing for a workout; you will find your ideal tension within the first few sessions.

  • Tighter is not more supportive. An over-tightened brace can cause circulation issues and interfere with recovery.

  • If the brace is causing pain, it may be too tight or the wrong size. A correctly sized brace will feel comfortably snug and supportive without being excessively tight.

Brace use is typically most frequent during flare-ups or planned increases in activity. As your strength improves and symptoms settle, you may find you need it less. That’s a good sign, it means the underlying issue is being addressed.

The most common mistake people make is using a brace to push through pain at full intensity rather than as one part of a structured recovery plan. If your pain is getting worse despite bracing, reduce your activity and speak with your athletic trainer or physician.

How Long Does Patellar Tendonitis Take to Heal?

Recovery timeline for patellar tendonitis can vary depending on how early the condition is addressed and how closely you follow a structured treatment plan. Recovery depends on activity level, adherence to rest, and underlying strength imbalances.

In general, most people can expect:

  • Mild cases (early-stage irritation):

    3 –12 weeks with activity modification, targeted stretching, and gradual return to movement.4

  • Severe cases (chronic pain or tendon degeneration):

    6 – 12 months, often requiring guided physical therapy and a carefully monitored return-to-sport plan.4

It’s important to understand that patellar tendonitis is not just about reducing pain, it’s about restoring the tendon’s ability to handle load. Returning to activity too quickly can reset your progress and prolong recovery.

When to See a Doctor

Patellar tendonitis can be managed on your own in many cases, but there are situations where professional evaluation shouldn’t wait. Seek professional medical care if you experience:5

  • Pain that worsens despite reduced activity and rest

  • Swelling that becomes constant rather than tied to activity

  • Symptoms that don’t improve after modifying your training and giving the tendon time to recover

  • A visible bump or targeted pain on the shin bone where the tendon attaches

  • Inability to straighten you knee

Athletic Trainer’s Take

Patellar tendonitis is an easily managed injury if you treat it early and properly; it’s also a frustrating injury when you push through the symptoms and don’t address root causes. The pain just below the kneecap is your tendon telling you the workload has exceeded its capacity, and the fix isn’t just rest, it is rebuilding the strength and movement patterns that caused the imbalance in the first place.

Bracing is a real tool in that process. A patellar tendon strap may help provide relief during activity and give you the confidence to move through the early stages of recovery. But keep in mind it works best when you’re also doing the stretching, building strength, addressing the muscle imbalances, and gradually working back to full load under your physician’s guidance.

The goal isn’t to get back to your sport as fast as possible. It is to come back stronger than when you left and stay there.
